Pharmaceutical Assistance |

for HAI will also be eligible for medication assistance. Steps for Pharmaceutical Assistance Program: 1. Instruct the client income documentation must be available at the time of appointment with a Health Access representative before any application will be processed. (See HAI check list) 2. Complete referral form and fax to Health Access with copy of current medications. Check box “Pharmaceutical assistance program”. 3. Give the patient a copy of the checklist. 4. After the referral is made a Health Access representative will contact the patient to set up an appointment. 5. Remind the client not all medications are available through the PAP. The patient may be responsible for some medications. As a rule, HAI is unable to help with immediate needs. Please allow 6-8 weeks to process any medication requests. However, if there is no other resource for the immediate need and the need is critical, please call the HAI office. |
