Physician Referral Information |

| Patients
eligible for HAI services are also eligible for specialty referrals that
are available within the physician referral network. Not all specialties
are represented within the network. Out of network referrals should be
made through the physician’s office and payment for these services is
the responsibility of the patient. Steps for acquiring a specialty referral: 1. Refer to the physician network master list to see if the requested services are available. If you are not sure please call the HAI office to inquire if a specific service is available. 2. Complete the HAI referral form and fax to Health Access with a copy of any pertinent medical records, labs or xray reports that need to accompany the patient to their specialty referral appointment. Please indicate on the referral form any important information needed when making the appointment (diagnosis, reason for appointment, timeframe for appointment, etc… 3. A member of the HAI staff will make the referral appointment and update your office as to the outcome. 4. Client must present a valid HAI ID card at the time of their appointment. If this card is not presented at the time of service the patient will be responsible for the bill. This procedure is to be used for referrals to any physician (specialty or primary care provider). |
