Community Supporters

• complete the Provider Form and fax it back to HAI at 770-287-0722. 
• Or you can complete the online Physician Form.

You can choose the number of patients you will see each month at no charge.  Then, we will enter you into our database of community resources and call on you when we have an appropriate referral.  We will help patients get the medications and ancillary services they need if they are not available through your office.

HAI keeps track of the value of services provided and will stay in contact with you about the patients follow-up care. Please ensure that your office submits a “health insurance claim form” so we can track the service provided and the value of the service.  You can do this by faxing the form to the office or submitting the claim to the address shown on the back of the clients Health Access ID card.

• Invite us to speak at your civic or church organization.  Our physicians can provide informative presentations about the plight of Hall County's working poor and how HAI fills the gap.
• Volunteer with HAI staff or at partnering agencies. Fill out the volunteer information form and submit online.
• HAI is a 5013c non-profit. Tax deductible donations can be sent to:  PO Box 907446, Gainesville, GA 30503.
